The Health Professional (Pre-Med) Pathway is a program offered through Corner Canyon High School. This pathway is designed for students who are interested in a medical profession or a pre med path through college. This course work will prepare students for the rigorous courses they will face later in their schooling. The courses are tough but the rewards are great. Students can earn two honor cords, 3 CTE state certifications, the highly coveted pre-med medallion, in addition to being a white coat recipient. In order to fulfill these requirements, several tracking sheets are provided below! Every student in the pathway will have a file with Mr. Anderson where we will track your progress. The Volunteer Hours Tracking Sheet must be kept in your file except when you are out fulfilling hours. If Mr. Anderson or Ms. Hanks get a tracking sheet from a senior they have never seen before, it WILL NOT be accepted!
***New forms have been uploaded for the 2019-2020 School year. They have one change, the new forms specify that HOSA years MUST BE ACTIVE. This means that a student must attend 50% of meetings or more each of their two years as a HOSA member. We take roll and have the ability to check if you are active or not. If you don't attend or if you fail to get on the roll for an activity that may disqualify you for the award!***
As a senior, we will have a meeting at the beginning of 3rd quarter to go over the application requirements. You will need to turn in a completed application to the office by 4/15/22. Incomplete or late applications WILL NOT BE CONSIDERED, you need to take the time to insure your application is correct and complete by the application date. This section will describe what needs to be turned in and how it must be turned in to receive this award. This year, we will be using an independent reviewing committee, please do not come to Mr. Anderson, Ms. Hanks, or Mr. Hone with questions regarding the outcome of your application. We have nothing to do with the final decisions.
Step 1 - Print the appropriate forms, You need to turn in the following forms:
The entire application must be filled out and an unofficial transcript must be obtained and included.
Fill out all of your grades on the appropriate spaces on the application. Write down your grades for each quarter for each class, the grades will be verified with the transcript provided.
Include all of your electives, only one is required but include all of the electives you have taken.
Mark the years you were an active member of HOSA (attended 50% of activities or more). If you weren't an active member the year WILL NOT COUNT, we have kept roll at each activity, we will go back and check each year!!!
Write down your full name as you would like it on your white coat.
Pick your white coat size. I typically go up one size, for example, if you are a small with normal clothes get a medium white coat.
Sign the Application.
Step 3 - Get a copy of your transcript (an unofficial transcript will be ok)
You can get a transcript by requesting a copy from the registrar's office (across from the main office where the counselors offices are). Your transcript must be included in your application.

Step 4 - Complete your hours log.
There are three options here:
If you did a CTEC course that had clinical hours associated with it do the following:
Get your hours log from your CTEC course and attach it to your application and transcript. Make sure your hours are verified by your teacher.
If you did the internship at the high school.
You can fill out the Volunteer Hours Tracking Sheet and have Ms. Grinder verify those hours or your can turn in your tracking sheet from the course.
If you did your hours on your own.
Fill out the Volunteer Hours Tracking Sheet, you need 40 hours total, 18 of those hours must be clinical (in a medical facility). All of these hours must be verified or signed for by someone overseeing those volunteer hours.
Step 5 - Turn in your application to the office by the due date described in our meeting (4/15/22). LATE OR INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED OR CONSIDERED!!!
